What this form is for
The extended import form is the full version of the import template: unlike the simplified form, it covers every life-cycle module — A1–A3, A4, A5 (any resource, not just packaging waste), B1–B7 (use stage), C1–C4 (end of life), and D (benefits and loads). Use it whenever a product doesn't fit the simplified form's scope: it includes B1-B5 or B7 in use stage, has A5 flows beyond packaging waste, or needs a custom, justified end-of-life scenario rather than one of the pre-built ones.
Even in the extended form, you don't always have to build end of life from scratch: if the client has a use stage (other than just B6) but is happy to rely on One Click LCA's ready-made, generic end-of-life scenarios rather than a custom one, you can still use those — see section 4 below for how.

@HELP — version number and update history for the template. Worth a quick check if you're unsure whether you have the latest version.
@A1-A3 Data Collection Form — you've already gathered the client's recipe and consumption data using the data collection form covered in Chapter 1, so there's nothing to fill in on this tab except two figures, which feed live formulas in the DATA tab (see section 3).
DATA — the single tab where all life-cycle data for every module is entered.
INFO EPD Hub/EPD Hub NA, INFO IES, INFO NMD, INFO INIES, PEP — four versions of the text-and-declared-unit tab, one per target EPD programme/tool. You only fill in the one that matches the programme this EPD is being verified against.
@MASS BALANCE — a built-in sanity check comparing what went into the product against what left it, used just before finalizing.
@LIST — the source of all dropdown lists. Don't edit this tab.
Step 1: Enter the two figures on the @A1-A3 Data Collection Form tab
You've already collected the client's raw material, packaging, ancillary, waste, energy and transport data using the data collection form from Chapter 1 — so there's nothing left to gather here. On this tab, you only need to enter two figures:
Total annual 12-month production volume (kg)
Mass of the declared unit (kg)
These two numbers are pulled directly into the DATA tab's calculations (see the "12-month data?" toggle below), so they must be filled in accurately before you start entering rows in DATA — if either is missing, any row relying on annual data will show an error asking you to check the Data Collection tab.
Step 2: Enter everything in the DATA tab
The DATA tab holds one row per resource, for any module. Unlike the simplified form (limited to A1 Raw materials and A3 Packaging materials), the module dropdown here offers the full list: A1 Raw materials, A1-A3 Separate transportation, A2/A4 transport (and fuel variants), A3 Packaging/Ancillary/Energy/Waste/Process direct emissions, A5 Installation resources/waste/transport, B1 Use through B7 Operational water, C1-C4 End of life, and D Benefits & Loads.
For each row, fill in:
Module — select the life-cycle module/class this row belongs to.
Resource — the material or process name, the exact database datapoint name (see the end-of-life note below for an important exception).
LCA stage — only for "A1-A3 Separate transportation" rows: choose which of A1, A2 or A3 the transport belongs to.
Input quantity and Unit — the amount and unit of measurement.
12-month data?— this is the key toggle that decides how the quantity is converted:
Select Yes if what you entered is an annual total (typical for energy, ancillary materials, and waste). The sheet will automatically divide it down to a per-declared-unit amount using the annual production volume and declared unit mass you entered on the Data Collection Form tab.
Select No if what you entered is already a recipe-level, per-declared-unit amount (typical for A1 raw materials and A3 packaging, where you usually know the exact amount that goes into one unit of product).
Country, production losses (A1 only, whole number), comment, and classification, as relevant.
Transport — entered in the same row as the material it belongs to: distance and method for leg 1, and again for leg 2 if a second leg is needed.
End of life stage— required for "C1-C4 End of life" rows, with two ways to model it:
Custom, stage-by-stage: choose C1 – Demolition, C2 – Transport, C3 – Waste processing, or C4 – Final disposal, and add one row per resource/process for each stage you can justify with your own data.
Generic/pre-built scenario: choose Construction Group, and in the Resource column type the exact name of one of One Click LCA's ready-made end-of-life scenario datasets (the same kind of named scenarios used in the simplified form, e.g. an "A5 x EoL … EU scenario" or "C1-C4 …" dataset). Because the Resource column is matched against the software's own database, typing that exact name is enough — you don't need to split it into C1/C2/C3/C4 yourself. This is the option to use when the client wants a use stage modelled but doesn't need or want to justify a custom end-of-life.
Output mass type — only for waste-flow rows (A3, B3–B5, C3): tells the software what happens to that output (reuse, recycling, energy recovery, disposal, or none).
Use for +A1/+A2/TRACI — only relevant for A1–A3 rows if the datapoint has variant answers per standard; otherwise leave as "All".
Coproduct allocation — only for A1 raw materials and A3 energy rows, if applicable.
Biogenic carbon balancing and Energy balancing — optional automation dropdowns (e.g. "Balance in EOL (C3, C4)", "Balance in A3", "Balance in Installation (A5)"); if you choose a balancing option that references end of life, add the % split in the columns provided next to it. For guidance on how to approach the balancing, please refer to this Help Center article.
Step 3: Fill in the matching INFO tab
Use whichever of the four INFO tabs matches the EPD programme/tool this project is being verified under (EPD Hub/EPD Hub NA, IES, NMD, or INIES/PEP) — you only need to complete one of them, not all four.
The structure mirrors the simplified form's INFO tab (manufacturer information, product identification and classification, raw material composition, declared unit) but goes further, since the extended form supports the full lifecycle:
A1–A3 product stage, A4–A5 construction stage, and — where module B is used — the full B1–B7 use-stage description fields (maintenance, repair, replacement, refurbishment, energy and water use).
C1–C4/D end-of-life description and allocation/averaging sections.
EPD visuals, and (depending on the tool) EPD programme, PCR, and standards/verification sections.
One field worth double-checking: mass of the declared unit is entered both in the Data Collection Form tab (to drive the annual-to-per-unit conversion in DATA) and in the INFO tab (as the declared answer for the EPD itself, used by the mass balance check in section 6). Make sure both figures match.
Step 4: Check the @MASS BALANCE tab
Before finalizing, review this tab — it automatically compares total inputs against total outputs for each life-cycle group (e.g. A1 raw materials vs. C1-C4 end of life, A3 packaging vs. A5 installation waste, A3 ancillary materials/A1 production losses vs. A3 manufacturing waste), and flags whether each pair is within an acceptable ±1% range. It also cross-checks the declared unit mass in the INFO tab against the total A1 raw material mass entered in DATA. Note that this checker doesn't account for module B, so if the use stage is included, apply the same input/output logic manually to those rows.
Before you submit
Confirm the Data Collection Form tab's production volume and declared unit mass are filled in before relying on the "12-month data? = Yes" conversion anywhere in DATA.
Confirm every C1-C4 End of life row either has a specific stage selected (with your own justified data) or uses "Construction Group" with an exact, valid pre-built scenario name as the Resource.
Confirm you've filled in only the one INFO tab that matches this EPD's target programme/tool.
Review the @MASS BALANCE tab and resolve any pair flagged outside the ±1% range, remembering to check module B rows manually if used.
Double-check that the declared unit mass matches between the Data Collection Form tab and the INFO tab.
